      Job summary

      This is an exciting opportunity for an experienced ACP/ANP to join a forward thinking practice that is constantly seeking to expand and improve our service-delivery. The successful applicant will be supported by a high-functioning Partnership and experienced clinical and administrative team.

      Main duties of the job

      • Make professionally autonomous decisions for which he/she is accountable.
      • Deliver on-the-day, acute clinical access for a wide range of presentations.
      • Support with GP administrative duties such as fit notes and medication queries.
      • Carry out home visits for acute presentations.
      • Support the daily triage team to answer incoming queries.
      • Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision making, to establish a diagnosis and management plan.
      • Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/ reports.
      • To prescribe safe, effective and appropriate medication as defined by current legislative framework.
      • Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
      • Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.
      • Work closely with the doctors and administrative managers in the setting up and/ or improving practice systems for monitoring/ measuring performance against Clinical Governance and Quality.
      • Keep up to date with pertinent health-related policy, work with team to consider strategies for implementation.

      Clinical Care Delivery

      • Provide patients the choice of direct or remote access to an ACP/ANP, both in general practice, via digital media, telephone or home setting for the general practice population.
      • Make professionally autonomous decisions for which he/she is accountable.
      • Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan.
      • Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret finding/ reports.
      • To prescribe safe, effective and appropriate medication as defined by current legislative framework.
      • Provide safe, evidence-based, cost effective, individualised patient care.
      • Offer a holistic service to patients and their families, developing where appropriate an ongoing plan of care/ support, with an emphasis on prevention and self-care.
      • Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.
      • Refer patients directly to other services/ agencies as appropriate.
      • Participate in developing and delivery specialist primary care services such as long term disease management and ensure relevant frameworks are being delivered.

      • Record accurate consultation data in patients records in accordance with the latest professional guidance and other pertinent standards.
      • Keep up to date with pertinent health related policy and work with the practice team to consider the impact and strategies for implementation.
      • Work collaboratively with colleagues within and external to the practice.
      • Demonstrate leadership.
      • Pro-actively promote the role of the ACP/ANP within the practice and externally to key stakeholders and agencies.
      • Participate in multi-disciplinary protocol and patient group directions development.
      • Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff.

      • Participation in an annual appraisal, including taking responsibility for maintaining a record of own personal and/or professional development.
      • Attending relevant professional courses and study days.
      • Ensure they are carrying out regular clinical audits of prescribing, consultations and referrals.

      Communication:

      The post- holder should recognise the importance of effective communication within the team and will strive to:

      • Communicate effectively with other team members, including the Community team, Social Services and staff from Voluntary Organisations as required.
