What to Expect (Job Responsibilities):
- Communicate consistently with the Account Management and leadership teams regarding new business plans and updates
- Provide customer service to patients, ensuring inquiries are resolved effectively
- Complete pre-surgery and confirmation calls to gather necessary patient information
- Process referrals accurately and collaborate with ADONs and Patient Access Representatives
- Facilitate coordination of services to support physicians, patients, caregivers, and the community

What is Required (Qualifications):
- Must possess excellent verbal and written communication skills
- Minimum of one year of experience in hospice admissions preferred
- At least two years of experience in healthcare data; LPN preferred
- Previous experience in customer service
- Experience in home health care, durable medical equipment, or skilled nursing facility preferred

How to Stand Out (Preferred Qualifications):
- Experience in hospice admissions
- Familiarity with EMR systems
- Ability to build rapport with clients and account management
- Experience working independently and as part of a team
