- Current registration as registered nurse
- Educated to Masters' degree level in relevant speciality
- Evidence of post registration development in autonomous practice/clinical skills (e.g Masters/ACP Masters/PG Cert/PGradDip)
- Completion of relevant extended role scope competencies
- Non-medical prescribing qualification &/or evidence of qualification in prescribing specialist equipment, care, therapies, lifestyle advice and care
- Leadership and management qualification or equivalent
- Experience of a senior professional role in relevant clinical speciality with highly specialised knowledge relevant to the position.
- Range of assessment methods as appropriate (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments).
- Knowledge of Quality Improvement methodologies and experience of successful delivery in practice
- Working knowledge of safety and governance frameworks and standards (including confidentiality/freedom of information/Caldicott regulations/CQC essential standards)
- Clinical role model/advocate/educator/coach for care delivery and development
- Collaborative working with a range of multi-agency and interprofessional resources
- Continually developing practice in response to changing population health needs and future developments in national guidance relevant to the clinical speciality
- Collaborative working with a range of multi-agency and interprofessional resources
- Advocating for and contributing to a culture of organisational learning
- Excellent communication skills, written and verbal, to enable communication with a wide range of stakeholders and for clinical purposes, negotiation, leadership and influence. This includes managing highly complex communications where there may be barriers to understanding.

ADVANCED CLINICAL PRACTICE DIABETES SPECIALIST NURSE - BAND 8A
Are you looking for a rewarding new opportunity as an Advanced Clinical Practice Diabetes Specialist Nurse (DSN)?
We have an exciting opening for an experienced and motivated candidate to join our Hounslow Community Diabetes Service (HCDS) based at Heart of Hounslow Centre for Health.
This is a clinical leadership role in the team providing scope for innovative work with Primary Care Partners, Acute Providers and Community Nursing development. Within the team, supporting the development of the DSN role including with prescribing.

Our team consists of DSNs and Diabetes Specialist Dietitians (DSD), with support from a Consultant Diabetologist.
The service also employs 3 PCN DSDs who each work 1 day in the HCDS.
The DSDs deliver the X-PERT education program which has won national awards for patient outcomes in the last three years.
There are opportunities for flexible working as you will be provided with the tools to deliver virtual consultations, but you will also be expected to travel around the Hounslow Borough visiting GP practices as required.
Support will be provided by the HCDS Manager and the Divisional Manager for Planned Care.
Further opportunities for training and personal development will also be available for this role.

Main duties of the job
Main duties of the job
As part of our team, your role will:
Lead the DSN service supporting best practice in Type 2 Diabetes in the community.
Work closely with patients, other specialists including the Consultant DSN employed by NWL ICB, Primary Care colleagues, and Community Teams to improve the health outcomes of people with Type 2 Diabetes.
Lead and develop the partnership working with Primary Care and the Acute Hospital to maximise patient outcomes.
Provide support to the Consultant DSN in providing GP training in Diabetes on PITstop courses.
Assist with supporting diabetes-related projects within the trust such as insulin education for Health Care Support workers; representation at PCN level supporting Fuller Report working groups to improve health inequalities.
