Job summary
Are you Advanced Nurse Practitioner who holds a Digital Badge or is currently on the pathway working towards this?
This is an exciting opportunity to join the team at North East Derbyshire PCN as an Advanced Nurse Practitioner. The role is at the front line of the NHS/primary care, seeing acute presentations at GP surgeries. You will be responsible for assessing these patients, taking a history and examination and developing management plans whilst identifying any red flags that may need onward referral. Some cases may need follow up, and this will give you the opportunity to develop continuity and build relationships with patients and their families over time.
The successful candidate will be well supported in the role, all of our practices have experience of developing nurse practitioners "in house" and encouraging independent practice and there will be a GP available to discuss cases with if you need support. Whilst experience in prescribing would be preferred, we are also happy to support with the Non-Medical Prescriber's course if this is needed. We would also actively support any development of specialist interests that would benefit the PCN.
For a further discussion around the role please contact Sarah Buckley/Dr Tom Martin.
Main duties of the job
The postholder will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The practitioner will need to prioritise the needs of patients, making any necessary referrals for investigations in the appropriate manner.
The practitioner will be supported in their role by GPs within the practices within the PCN. There will be opportunities for upskilling and further training within the role if necessary. We anticipate that the successful candidate will be based at one or two practices within the PCN, subject to discussion around working patterns. Although we have a full time vacancy, we may consider part-time or job share.
About us
NED PCN is an organisational group of four proactive GP practices in North East Derbyshire. The Practices are Barlborough Medical Practice, Killamarsh Medical Practice, The Springs Medical Partnership and The Valleys Medical Partnership.
NED PCN is a relatively small PCN with just over 40,000 patients. We feel that this makes us big enough to develop a successful team but small enough to be an approachable, friendly place to be.
The NHS Long Term Plan describes the prominent role Primary Care Networks play in delivering proactive, personalised and more integrated health and social care for their local populations. This will require collaborative working between organisations including GP practices, acute and community health, social care organisations and the voluntary and community sector.
*PLEASE NOTE THAT THIS JOB MAY CLOSE EARLIER THAN THE DATE ADVERTISED
Core practices opening hours of 8:00am 6:30pm Monday to Friday each week.

About the company
National Health Service (NHS) is the umbrella term for the publicly-funded healthcare systems of the United Kingdom (UK). The founding principles were that services should be comprehensive, universal and free at the point of delivery—a health service based on clinical need, not ability to pay. Each service provides a comprehensive range of health services, free at the point of use for people ordinarily resident in the United Kingdom apart from dental treatment and optical care.
