ADVANCED NURSE PRACTITIONER - P/T - Fulltime. WITH FLEXIBILITY EQUIVALENT BAND 8

We are a well organised, supportive, innovative GMS practice operating from a purpose built surgery in Kidderminster.
We are looking for an Advanced Nurse Practitioner to join our friendly and supportive team. The ANP will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long term conditions in line with existing practice and national policies and guidelines.
You will have experience of carrying out ANP duties in General Practice and will have a non-medical prescribing qualification.
Salary: dependent on experience BUT BAND 8 EQUIVALENT

Main duties of the job
The ANP will be part of a team of 3 ANPs and will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long term conditions in line with existing practice and national policies and guidelines. 15 minute F2F appointments and a mix of telephone and face to face.

Key duties and responsibilities
1. Clinical Practice
a) Assess patients’ with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
b) Clinically examine and assess patient needs from a physiological, psychological and social perspective, and plan clinical care accordingly.
c) Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
d) Diagnose and manage acute, integrating both drug- and non-drug-based treatment methods into a management plan.
e) Perform safe triage of patients when necessary via telephone or face to face.
f) Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
g) Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
h) Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
i) Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care where appropriate.
2. Communication
a) Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
b) Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
c) Anticipate barriers to communication and take action to improve communication.
d) Maintain effective communication within the organisation and with external services/providers.
f) Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
3. Delivering a Quality Service
a) Recognise and work within own competence and professional code of conduct as regulated by the NMC.
b) Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
c) Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
d) Deliver care according to NSF, NICE guidelines and evidence-based care.
e) Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
f) Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
g) Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
h) In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.

a) Undertake mentorship for more junior staff, assessing competence against set standards.
b) Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences).

d) Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.
