      Job summary

      Bramley Village Health and Wellbeing Centre is currently looking to recruit a full time Advanced Nurse Practitioner to join our team at our Bramley site.

      We are a multidisciplinary practice that includes 6 GPs, GP Registrar, 4 ANPs, Clinical Pharmacists, Practice Nurses and Health Care Assistants.

      The main duties of the role would include the completion of undifferentiated patient clinics that are a mixture of pre-booked and on the day triaged appointments.

      Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients within face to face and telephone consultations ensuring the use of evidence-based practice. The ANP will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and active participation in practice research activities to promote a culture of learning and continuous improvement within the practice.

      Main duties of the job

      Clinical Responsibilities:

      Patient Care: Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.

      Autonomous Practice: Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.

      Case Management: Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.

      Prescribing: Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.

      Diagnosis and Treatment Planning: Utilise clinical expertise to diagnose health conditions based on face to face and eConsultations. Prescribe medications, suggest lifestyle changes, or recommend further diagnostic tests as appropriate.

      Referral Coordination: Refer patients to other healthcare services when more detailed investigations or specialist care is needed, ensuring a seamless transition from virtual to in-person care.

      About us

      Bramley Village Health and Wellbeing Centre is one of three surgeries across Leeds that are managed by the Fuller and Forbes Healthcare Group. As well as the two other sites in Leeds, Cottingley and Middleton Park, the healthcare group currently operates a total of 17 surgeries in Gateshead, Burnley and Devon providing care to 120,000 patients.

      Our 3 Leeds based surgeries provide GP services to our fast growing population of 17,000 patients. Therefore, this role presents an exciting opportunity for any suitable individual who enjoys the demands and rewards of a fast-paced and continually expanding environment.

      The organisation is patient-driven and has used patient feedback in the past to develop innovative services such as a bespoke smart phone app, patient-centred practice websites and a bespoke e-consultation service. The econsultation service is accessible through both the smart phone app and websites.

      • To provide continuing care to patients across the whole practice population
