      Sentara Medical Group has a full-time opening for an APP (NP or PA) to join our Ambulatory Remote Patient Monitoring team in providing primary care.

      This is a new program in partnership with HealthSnap that is focused on preventive measures to help patients manage chronic conditions such as high blood pressure, diabetes, etc. with a goal of preventing admissions and readmissions to the emergency department.

      Position Highlights:

      • 100% remote
      • 8am-5pm, no nights or weekends
      • Provides collaboration in a team-based environment with other primary care providers and HealthSnap employees

      Minimum Requirements:

      • Active NP or PA
      • Experience strongly preferred
      • Applicants may live in the following states: Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin

      Job Summary

      Qualifications:

      MLD - Master's Level Degree: Nurse Practitioner (Required), MLD - Master's Level Degree: Nurse Practitioner (Required)

      Advanced Cardiovascular Life Support (ACLS) - Certification - American Heart Association (AHA) RQI, Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI, Licensed Nurse Practitioner (NP) - Certification - Other/National, Licensed Nurse Practitioner (NP) - Nursing License - North Carolina, Licensed Nurse Practitioner (NP) - Nursing License - Virginia Department of Health Professionals (VADHP), Physician Assistant License (PA) - State License - North Carolina Medical Board (NC), Physician Assistant License (PA) - State License - Other/National, Physician Assistant License (PA) - State License - Virginia Department of Health Professionals (VADHP)
