      SSM Health has an exciting opportunity to join our Virtual Behavioral Health integration team. This is a transformational approach to identifying patients early and providing them the access and support they need to address their behavioral health issues BEFORE they become unmanageable. This position will work with patients COMPLETELY VIRTUALLY.       Job Summary:

      Works in a call-center type environment, engaging with patients virtually via phone and encourages them to take an active role in their mental and physical health by finding solutions and the tools necessary to make healthy life choices and adopt lifelong healthy behaviors.

      Job Responsibilities and Requirements:

      PRIMARY RESPONSIBILITIES

      • Engages a behavioral health patient population by providing support, encouragement, and assistance in working towards improved mental and physical wellbeing.
      • Implements a range of skills such as coaching and motivational interviewing and other skills (e.g. , goal setting, conflict-resolution, and problem solving ) .
      • Partners with patients to create self-directed, sustainable change, aligned with the patient’s values to promote health and wellness and enhance well-being.
      • Encourages patients to connect with community resources, behavioral health providers and other services to encourage long-term connections that will maintain overall well-being .
      • Provides customized, evidence-based patient support , in a variety of areas (e.g., depression, anxiety, grief, weight management/exercise, tobacco cessation, stress reduction, and chronic disease self-management ) .
      • Encourages the patient to participate in shared decision making with his/her doctor when faced with multiple treatment options. Helps the patient review o ptions and evaluate personal preferences and values related to likely treatment outcomes, with the goal of preparing the patient to discuss the risks and benefits of each option with the physician .
      • Receives inbound calls and places outbound calls, transfers calls to appropriate team members, and answers patient questions about services offered.
      • Performs other duties as assigned.


      EDUCATION

      • Bachelor's degree or equivalent years of experience and education


      EXPERIENCE

      • Six months’ experience in a Health , Wellness or Lifestyle Coaching

      PHYSICAL REQUIREMENTS

      • Constant use of hearing to receive oral communication and hear alarms, malfunctioning machinery, etc.
      • Constant use of vision for distances near (20 inches or less) and far (20 feet or more).
      • Constant lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
      • Frequent use of speech to share information through oral communication.
      • Frequent lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
      • Frequent standing, twisting and walking.
      • Frequent use of vision to judge distances and spatial relationships and to identify and distinguish colors.
      • Occasional bending, stooping, climbing, kneeling, sitting, squatting, reaching, gripping and repetitive foot/leg and hand/arm movements.
      • Occasional lifting/carrying and pushing/pulling objects weighing over 50 lbs.
      • Occasional keyboard use/data entry.
      • Occasional use of smell to detect/recognize odors.
      • Occasional driving.
      • Rare crawling, running and lifting/moving patients.
