What to Expect (Job Responsibilities):
- Collaborate with At-Home providers and Psychiatric Consult to address patients' behavioral health needs
- Facilitate goals of care and advance care planning discussions with patients and families
- Manage referrals for specialized services and track patient progress through a shared behavioral health registry
- Screen and assess patients for mental health and substance use disorders, developing care plans as needed
- Provide short-term counseling and evidence-based treatments within the Collaborative Care Model

What is Required (Qualifications):
- Master of Social Work degree from an accredited school of social work
- Clinical Social Worker License (or local equivalent)
- Experience performing comprehensive biopsychosocial health assessments
- Ability to collaborate effectively in a team setting and support patients with significant health issues
- Proficient PC skills and US work authorization

How to Stand Out (Preferred Qualifications):
- Familiarity with brief, structured intervention techniques such as Motivational Interviewing and Behavioral Activation
- Demonstrated ability to work in a variety of environments, including patients' homes
- Experience with assessment and treatment planning for mental health and substance use disorders
