      What to Expect (Job Responsibilities):
      - Maintain ongoing caseload of individuals using evidence-based approaches to promote engagement and health goals
      - Provide social support navigation to members, understanding their hesitancy or distrust of the healthcare system
      - Conduct telephonic and SMS outreach to ensure timely follow-up with members
      - Collaborate with the care management team to develop Health Action Plans
      - Assist with delegated tasks and collaborate on care issues with the Enhanced Care Management team
      - Meet monthly encounter metrics for compliance with health plan regulations
      - Break down barriers to ensure individuals' continuation with the program
      - Assist individuals in securing connection to community supports and coordinate physical care management appointments
      - Utilize online platforms to collaborate with team members and carry out daily tasks

      What is Required (Qualifications):
      - 3+ years of general work experience, preferably in case management
      - Physical location in/near Merced, California
      - Reliable transportation for engagement at various locations
      - Quiet, HIPAA compliant, and internet-connected workspace for remote work
      - 1+ year(s) of case management experience
      - Bilingual in English and Spanish
      - Strong understanding of cultural fluency
      - High degree of empathy
      - Ability to work collaboratively in a multidisciplinary team
      - Organizational skills
      - Ability to remain patient in challenging situations
      - Strong technical skills and comfort with technology innovation
      - Valid driver's license and auto liability insurance

      How to Stand Out (Preferred Qualifications):
      - Demonstrated experience working with individuals experiencing complex chronic needs, homelessness, or mental illness/substance use disorder
      - 2+ years of case management experience
      - Experience with motivational interviewing
      - Knowledge of medical terminology
      - Zest for problem-solving and thinking outside the box
      - Detail-oriented and organized self-starter
      - Reliable and comfortable in an ever-changing environment
