What to Expect (Job Responsibilities):
- Process incoming referrals and schedule clinical visits for homebound older adults
- Conduct outreach and inbound calls to enroll patients and arrange visits with clinicians
- Communicate with payor clients regarding patient engagement and scheduled visits
- Create and manage clinician schedules using mapping tools
- Track clinician travel mileage and support patient care coordination with other departments

What is Required (Qualifications):
- High School Education or GED preferred but not required
- 6 months to 1 year of relevant scheduling experience
- Bilingual in Spanish required
- Strong interpersonal skills, including excellent written and verbal communication
- Ability to manage and maintain confidential information
