Gundersen Health System is hiring a part-time Business Health Resource Navigator in Arcadia, WI . This is your opportunity to create a program facilitating Pilgrim’s Pride employees’ access to essential healthcare.

What's Available?

• Part time, 10-15 hours/week to start - there is a potential to expand hours as this program grows

• Schedule: hours can vary based on business needs

• Location: Onsite at Pilgrim's Pride in Arcadia, WI, with ability to have some hybrid availability after training

What Will You Do?

• Work with Pilgrim's Pride employees to provide education, direction, and support regarding how to access healthcare services at Gundersen Health System.

• Build and maintain strong relationships with key stakeholders, HR professionals and community partners.

• Work with Internal and External partners to advocate for patient needs and make recommendations to enhance the coordination of care.

• Develop, maintain, use, and share a comprehensive, reliable, and accurate community resource database.

• Provide outreach and education on access to healthcare to patients and families to improve health and population health outcomes.

What Do You Need?

• 2 years of recent experience in a healthcare or human service-related field

• Experience with resource navigation or case management experience is a plus

• Excellent communication and organizational skills

• Valid driver’s license and reliable transportation

• Bilingual, Spanish & English speaking, preferred but not required
