What to Expect (Job Responsibilities):
- Conduct telephonic clinical assessments using approved medical protocols
- Recommend appropriate levels of clinical care based on clinical judgment
- Engage and outreach to members regarding health programs
- Complete clinical eligibility assessments
- Identify health conditions or social gaps affecting member care

What is Required (Qualifications):
- Undergraduate degree or equivalent experience
- Unrestricted LPN or LVN license in the state of residence
- Unrestricted LPN or LVN license in the State of California
- Experience in outpatient clinical, ICU ER, PCU, or step-down settings
- Call center experience and computer proficiency (Microsoft Word, Outlook, and internet)

How to Stand Out (Preferred Qualifications):
- Experience with Electronic Medical Records (EMR)
- Recent experience in Case Management
- Bilingual in Spanish
