      Job summary

      The apprenticeship post is a fixed term, developmental role over 3 years, integrating academic and work-based learning towards successful completion of a BSC (Hons) degree in Occupational Therapy and recognition by the Health Care Professions Council (HCPC) as a registered Occupational Therapist.

      You will work under the direction and support of the Team Leader who is a registered Occupational Therapist to undertake assessments and provide individual treatments to enable patients to live and Age Well at home.

      You will continuously develop your skills and abilities as you progress through the degree programme so that on completion you have the level of required competence to meet professional standards as a registered occupational therapist.

      At the end of the programme the apprentice will be encouraged to apply for Band 5 occupational therapy posts within NHFT.

      Please note if you are non-UK national to be eligible for the apprenticeship you will need to have been ordinarily a resident in the UK and Islands for at least the previous 3 years on the first day of the apprenticeship and have permission from the UK government to live and work in the UK until at least September 2028 (not for education purposes) or have obtained pre-settled or settled status under EUSS.

      Main duties of the job

      Description of role:

      As a Community Care Coordinator you will need a range of experience, including hands on clinical care in a health setting, as a Health Care Assistant or similar, and be passionate about providing effective support to our older population within the community and in care homes.

      A good understanding of services available both within the community and hospital and an interest in frailty.

      The ability to develop close working relationships with other health, social care and volunteering partners is an essential aspect of this role.

      To build on existing networking opportunities. Liaise with GPs and practice teams to identify persons who are elderly, frail or who have long term health needs and support.

      Use provided templates and tools to undertake core assessment elements for inclusion in the care plan.

      Identify where technology can be utilised to support continued independence or to aid remote monitoring of health and wellbeing.

      Communicate effectively and sensitively and use language appropriate to a patient and carer/relative's condition and level of understanding.

      Effectively use all methods of communication and be aware of and manage barriers to communication.

      Provide information to patients, their carers and/or relatives on behalf of the team

      • Has 5 GCSES including Maths and English at Grade c/4 or above
      • Has the qualifications needed to do the course
      • Basic understanding of the role of Occupational Therapists in healthcare
      • Good Written and verbal communication
      • IT skills with microsoft programmes and work university portals

      Desirable

      • Experience of a health and social care setting
      • Experience of working alongside occupational therapists
      • Experience of supporting patients living in the community
      • Has already an offer from the University of Northampton

      Experience
