Job summary
Benfield Park Medical Group is recruiting on behalf of Newcastle East PCN, one part time (18.75hrs), Care Home & Frailty Specialist Nurse.
Main duties of the job
The successful applicant will be working in a highly autonomous fashion delivering high quality care liaising between nursing home GPs and other specialists in the multidisciplinary team.
The post is largely working in a local care home linked to the PCN and assisting PCN practices with their elderly housebound and frail patient caseloads.
We are looking for a practitioner who has experience or an interest in the management of frail elderly patients, patients with complex needs and/or patients requiring palliative and end of life care.
Essential skills and competencies would be around the ability to undertake a comprehensive assessment of the older adult, including taking an accurate history of their condition and presenting problem. The ability to employ appropriate diagnostic interventions to support clinical decision making. The ability to formulate a plan of care based on findings and evidence-based standards of care and latest guidelines. The ability to follow up and evaluate a plan of care.
Training and mentoring will be provided fortnightly by a GP based at Benfield Park Medical Group to equip the jobholders with the necessary skills.
