      Description

      Autism Society of North Carolina is currently hiring a full-time, Care Manager. The ideal candidate will be passionate about providing support and coordination across physical health, behavioral health, and other unmet health-related resources. This role ensures that all health needs and referrals are effectively managed to improve overall health outcomes for our clients.

      This position is community-based and candidates must be based in the Triangle or close-by as client populations are local to Raleigh, and there are in-person components requiring staff to come to the office and to travel to client homes.

      Day to Day

      • Provide whole-person care management for individuals and families with special health care needs
      • Collaborate closely with care team, individuals and service providers
      • Educate the individual and families regarding service assessments, plans and providers
      • Conduct assessments and develop Plans of Care
      • Identify providers and submit referrals and authorizations, and monitor services for compliance with both internal and external providers
      • Mentor clients through challenging situations, utilizing motivational interviewing techniques
      • Implement crisis management strategies when necessary
      • Provide childcare assistance when needed

      Qualifications


      Minimum Qualifications:

      • Bachelor’s Degree in field related to health, psychology, sociology, nursing or other relevant human services area
      • Two years’ experience providing Care Management, Case Management, or Coordination to complex individuals with IDD/ASD.
      • Ability to work independently from a satellite office covering a region of several counties without an onsite supervisor
      • Valid driver’s license and reliable transportation
      • Preferred Spanish speaking
