What to Expect (Job Responsibilities):
- Coordinate and implement the transition of care/discharge plans for ambulatory patients
- Prioritize and coordinate care plans across the continuum with critical thinking and teamwork
- Communicate effectively with care providers, patients, families, and external vendors
- Ensure timely discharge through effective planning and coordination
- Manage multiple tasks and shifting priorities in a fast-paced environment

What is Required (Qualifications):
- High School Diploma or equivalent education/experience
- Minimum of 1 year of experience in utilization management and discharge planning
- Strong oral and written communication skills
- Excellent interpersonal and time management skills
- Intermediate PC and word processing skills
