Full-time position
8-hour, day shifts
No regular weekend rotation
Position supports clients in Hennepin County
Case Manager will visit with clients throughout the week and will also attend onsite meetings at the Commons or other sites
- Provides ongoing case management, needs assessment and service coordination across a continuum of care, services and settings.
- Develops individual treatment plan and goals.
- Identifies desired individual services and health practices that enhance the care plan.
- Facilitates communication among team members, program and outside providers, referral sources, payers and community support.
- Completes documentation and assures that records are maintained.
- May facilitate completion of discharge planning.
- Provides ongoing evaluation of client needs and status and adjust service plan and community supports as needed.
- Identifies barriers and develops strategies to overcome barriers.
- Provides coaching and information to client and family.
- Advocates for client needs.
- Participates in quality/performance improvement activities related to case management and improved outcomes for clients.
- May participate in multidisciplinary care conferences.
- Other duties as assigned.
- Bachelor's degree in Social Work, Nursing, Psychology, Education or Vocational Rehabilitation
- Experience in health care, human services, or rehabilitation
- Licensed Social Worker - MN Board of Social Work preferred
- Case Manager Certification preferred
- Valid Driver’s License required upon hire for AXIS and Clinic 42 employees - MN and WI residents must obtain a valid driver’s license in their state of residence
- Sedentary:
- Lifting weight Up to 10 lbs. occasionally, negligible weight frequently

Pay Range: $30.39 to $41.65 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
