      Description

      Work a 32/hr. reduced work week and get paid for 40 hours (pay adjustment will be reflected in our verbal offer).

      The Case Manager position support individuals and/or families in scattered house programs Housing and Urban Development. The Case Manager, Housing/Mental Health (TCM) will provide Rule 79 Adult Mental Health Targeted Case Management. Avivo’s Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma informed service delivery.

      PRIMARY RESPONSIBILITIES:

      • Build meaningful and purposeful client relationships with appropriate boundaries
      • Inform and educate others on eligibility for programs and services
      • Communicate appropriate referrals internally and externally
      • Utilize creative approaches to meet in person, regularly with clients and address their needs
      • Attempt 3 or more creative contacts per client if needed
      • Use creative strategies to engage in goal planning
      • Engage and build rapport with clients
      • Assess eligibility for program and services
      • Connect individuals to desired and necessary services
      • Develop Goal Plan
      • Consult regularly with supervisor about client situations.
      • Make appropriate referrals, monitor progress, and ensure follow-up
      • Actively participate in Avivo’s Undoing Racism plan
      • Immediately consult about any safety related concerns (for client and staff), mandated reporting concerns, and complaints from the client
      • Meet with all clients on caseload, at least monthly and complete a billable case note for each client

      Qualifications


      • Bachelor's Degree in a Human Services related field
      • 2,000+ hours of supervised experience working with adults with mental illness.
      • Ability to provide Rule 79 Adult Mental Health Targeted Case Management per DHS guidelines.
      • One or more years of experience working with single adults experiencing homelessness or similar experience.
      • Lived experience will be valued and considered as part of the experience requirement.
