      Summary

      The Case Manager (CM) I is responsible for the leadership and functioning of their assigned case load. Case Managers develop professional and empathetic relationships while providing clients with connections to appropriate housing, programs and resources through one-on-one Case Management that develop individualized case plans that promote client progression towards obtaining and maintaining self-sufficiency.

      Essential Functions

      • Provides ongoing intensive support to clients which can include assessing, evaluating, and coordinating services, crisis intervention and applications for benefits & referrals to community resources
      • Communicates and advocates to landlords to support clients obtaining and maintaining housing placement

      · Utilizes a strength-based / trauma-informed approach to services along with Motivational Interviewing techniques to encourage positive change.

      • Prepares, presents and timely documents client cases
      • Participates as a member of a multidisciplinary team that prepares and presents client case presentations
      • Assesses clients for employment, social security and disability insurance eligibility, gathers records and schedules treatment
      • Manages a case load of clients
      • Enters, monitors and updates client database

      Essential Functions Specific to Case Manager I of clients served by Rapid Re-Housing:

      • The Case Manager I for the Rapid Re-Housing Program provides direct and indirect services to a caseload of culturally diverse individuals, Families, and/or TAY (transitional-aged youth) clients, including comprehensive assessment, information and referral, case planning, education, advocacy and other supportive services.
      • Case Managers to be comfortable with entering the client’s home on a monthly basis to the unit is being kept up and no breaking of the lease is occurring. Case Managers to be comfortable with driving all throughout San Diego County.
      • Conducts lease reviews, and completes monthly budgets and reviews monthly Rental Assistance Plan with clients.
      • Completes once a month check ins with the Property Manager in-person or over the phone
      • Case managers working in the Scattered Site setting use agency vehicles to deliver individualized services, in the field, to our neighbors in the various communities where they reside

      • On time completion of assigned training and policies.
      • Performs other duties as assigned.

      Qualifications

      • Bachelor’s Degree from an accredited four-year college or university with major coursework in healthcare, psychology, sociology, social work, public administration, or a related field, and one (1) year of applicable job experience, or
      • Associates Degree with a focus on healthcare, psychology, sociology, social work, public administration, or a related field and two (2) years of applicable job experience, or
      • High school Diploma or GED and a minimum of a combined two (2) years of lived experience, and at least two (2) years of applicable job experience
      • Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations and Prevention and Diversion practices, as well as theory, principles and practices of homeless housing interventions, social services, case management, eviction prevention, shelter diversion, and crisis intervention.
      • Basic user of MS Office.
      • Ability to pass fingerprinting and background check upon employment.
      • Must have a valid California Driver’s License and be able to qualify for insurance coverage (except for CMI assigned to Day Center)
      • Participate in an annual Tuberculosis screening and/or other screenings when necessary.
