      Description

      While receiving leadership support from the Mission Team Leader, the Case Manager advances our mission by managing the intake, creation, and oversight of participant employment plans to ensure individualized services are consistently delivered with all partners following best practices and funding source requirements. The Case Manager will align all activities and service delivery in a manner consistent with the mission, vision, values, and brand of Goodwill NCW.

      Key Responsibilities

      • Manages intake, creation, and oversight of participant employment plans to ensure individualized services are consistently delivered with all partners following best practices and funding source requirements.
      • Assists participants in identifying job goals and implementing goal plans to help them obtain, maintain, and elevate employment.
      • Cultivates and maintains a positive, collaborative relationship with job coaches, partners, community-based organizations, educational institutions, businesses, referral organizations and employment and training organizations to determine needs and adapt service delivery to meet the individual needs of the persons served.
      • Advocates Goodwill NCW mission services to the community to create awareness and promote referrals.
      • Reviews all participants case notes and partners with job coaches to ensure timely and accurate data entry into Case Management System by team.
      • Communicates with all those involved to ensure skills instruction support meets the specific needs of each participant and discusses any urgent concerns, changes in job duties or employer/participant feedback that requires immediate attention.
      • Provides guidance to and refers participants to the areas of career exploration, job search, employment applications, resumes, interview preparation and job skills.
      • Maintains partnerships in community which may include employers, non-profits, and school districts to assist individuals in developing work skills and employment opportunities to reach their full potential.
      • Tracks and manages individuals’ progress towards independence, which can include managing services such as job coaching.
      • Completes accurate and timely documentation and data entry for participant files according to funding sources, Goodwill NCW, and CARF requirements.
      • Develops and maintains relationships with funding sources and submits accurate and timely reports according to the contract requirements of each funding source.
      • Monitors purchase orders weekly for each service provided and requests new purchase orders from funding sources as needed.
      • Job coaches as needed.

      Qualifications


      Education, Skills & Experience Requirements

      • Associate degree in related field, but applicable work experience may be substituted for degree requirement.
      • At least five years of experience in a similar field is preferred.
      • Previous case management/coaching experience is preferred.
      • A valid driver's license with an acceptable driving record is required to travel to multiple locations.
      • Possess a reliable vehicle with at least $100,000 each person, $300,000 each accident/occurrence auto insurance coverage is required.
      • At least an intermediate level of competency with Word, Excel, PowerPoint, SharePoint, and Outlook.
      • Knowledge and experience with Case Management Systems (CMS) is preferred.
      • Excellent written and oral communication skills.
      • Excellent time management and prioritization skillsandthe ability to manage multiple projects under time constraints simultaneously.
      • Strong skills in collaboration and connection to community.
      • Strong attention to detail.
      • Ability to adjust to the changing needs of the department and organization and willingness to jump in and help where needed.
