      Shift Schedule

      1st - 6:00 A.M. - 2:30 P.M.

      2nd - 2:00 P.M. - 10:30 P.M.

      3rd - 10:15 P.M. - 6:15 A.M.

      Off days will be two (2) days other than Saturday and Sunday

      Responsibilities:

      The individual assigned to this position is responsible for performing prescribed care and treatment of patients under the direction of professional nursing staff such as but not limited to.

      Assist nurse with treatments and simple dressing changes (i.e., aerosol treatments, creams, ointments, sprays, etc.)

      Perform direct care and treatment and monitors the health status of patients under the supervision and direction of professional nursing staff. Perform simple nursing measures such as but not limited to applying splints and positioning devices, making occupied and unoccupied beds, changing linens, taking vital signs, weights, recording intake and output, etc. Follow established performance standards and perform duties according to nursing service policies and procedures. Observe all facility safety, infection control policy and procedures.

      Provide the assistance necessary when giving residents the following activities of daily living nursing care: oral hygiene; bathing; dressing and grooming; hair and nail care; shaving; toileting; assistance with meals, nourishments, and transportation to meal service areas; and mobility. Assist in obtaining urine, blood, and other specimens. Respond to residents’ needs promptly and reports concerns and observations to supervisor.

      Participate in the development and implementation of the care plan as outlined by the nursing plan of care. Carries out and documents programs and activities as indicated on individual support plan.

      Assist medical and nursing personnel during minor procedures, and specialty clinics.

      Assist in general maintenance, cleaning and organization of physical plant, equipment, and supplies. Assist in maintaining and stocking inventory of medical, nursing, and central supplies.

      Perform all other related duties as directed by immediate supervisor or designee, including training.

      Knowledge, skills, and abilities:

      Must have the ability to perform the following physical tasks continually or frequently throughout the workday: climbing, grasping, kneeling, pushing/pulling, reaching above head, lifting at least 40 pounds, standing, stooping or bending at waist.

      Ability to plan, organize, and coordinate activities.

      Ability to follow behavior support plans.

      Ability to implement resident programs.

      Ability to communicate effectively verbally and in writing.

      Ability to establish and maintain effective working relationships with others.

      Minimum Requirements:

      Possession of an active/valid Certified Nursing Assistant License

      Complete all requirements to maintain license to include CEU’s.

      This position may be required to perform essential services before, during, and after an emergency per APD OP 2-0035.

      This position is critical to the safety and supervision of residents and may require the incumbent to work beyond their normal work schedule to maintain minimum coverage levels in the facility.
