      Description

      JOB SUMMARY: The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Bristol Hospice LLC, either directly or through coordination of care with other spiritual counselors.

      REQUIRED EDUCATION & EXPERIENCE:

      • Must be a graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree
      • Must have experience working with death and dying, individuals/family/caregiver
      • Hospice experience preferred

      REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

      • Must possess the ability to work as member of interdisciplinary group
      • Must demonstrate comfort in an interfaith setting
      • Must possess the ability to accept different lifestyles, cultures, beliefs and values
      • Must possess the ability to network with community, clergy and congregations
      • Must demonstrate knowledge of and commitment to hospice philosophy of care

      OTHER REQUIREMENTS:

      • Must be flexible in work hours

      ESSENTIAL JOB FUNCTIONS:

      • Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
      • Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
      • Provide direct spiritual care to patients and families/caregivers
      • Serve as liaison and support to community chaplains and spiritual counselors
      • Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
      • Document direct services and ongoing communication with community chaplains and spiritual counselors
      • Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
      • Provide consultation, education and support to interdisciplinary group on spiritual care
      • Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
      • Provide for funeral or memorial services for patients as requested.
      • Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team
      • Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers
      • Actively participate in quality assessment performance improvement teams and activities
      • Accept and perform other related duties and responsibilities, as required
      • WORK ENVIRONMENT:

      • Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
      • Must be dependable and flexible in work schedule

      • Must foster good working relationships
      • Must have a safe and functional automobile, valid drivers’ license and full automobile coverage
      • Must be willing to submit to a criminal background check prior to hire and annually thereafter
      • PHYSICAL ENVIRONMENT:

      • May be required to stand and sit for long periods
      • Must be able to hear and see with or without aid
      • Must be able to read and follow instructions as required
      • Must be willing to participate in a drug/alcohol screen prior to hire, if involved in a work-related automobile accident and if reasonable suspicion is reported.

      Qualifications


      JOB QUALIFICATIONS:

      • Rabbi Preferred
      • Graduate of accredited seminary or school of theology (BD, M.Div. or equivalent theological degree). Experience working with death and dying, individuals/family/caregivers.
      • Ability to work as a member of an interdisciplinary group.
      • Comfort in an interfaith setting.
      • Ability to accept different lifestyles, cultures, beliefs, and values.
      • Ability to network with community clergy and congregations.
      • Knowledge of and commitment to hospice philosophy of care.
      • Hospice experience preferred.
