      Job summary

      We have an opportunity for a passionate and determined Navigator to join the organisation on a fixed term basis. This is an opportunity to work as part of a charity committed to our vision of enabling everyone in our communities to achieve better mental health. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services. In your role as a CYP Population Health Navigator you will be part of an innovative population health management programme focused on providing more proactive identification and support to children and young people (aged 5-17) at risk of developing mental health condition. You will work across the West neighbourhood from community settings, providing holistic assessments, therapeutic interventions and overseeing care planning for children and young people. You will work as part of the PHM team and be supported by Tameside Oldham and Glossop Minds dedicated children, young people and families directorate and you will also connect with our Adults Navigator who supports the West district

      Main duties of the job

      You will be responsible for working with the PHM Care Coordinator in the identification, engagement and case management of at risk CYP, supported by data . You will complete holistic assessments and provide therapeutic interventions to the CYP. You will support CYP through the development of a robust care plan in collaboration with the PCN and wider Health and Social Care agencies to intervene early to maintain good health, prevent deterioration and ensure they are able to access services which meet their needs.. You will work with the coordinator to ensure the delivery and review of the care plan

      You will work from community bases across Oldham West and from the Mind office in Oldham . The role is highly relationship-driven and will involve building excellent relationships across our neighbourhood and the wider health and care system in Oldham West through a new integrated team as well as building strong, trusted relationships with the children and families this model seeks to support.

      You will work with the PHM Coordinator to monitor and track the progress of CYP, the effectiveness of interventions and the exploration of mechanisms to support new ways of working

      Be responsible for managing a caseload of young people accessing the service and overseeing their care planning

      Work alongside other staff and volunteers to ensure members of our community can access the services they need.

      Make decisions autonomously according to good clinical practice to ensure the safety of people accessing services and help identify areas for improvement.

      Maintain professional boundaries and work with partner organisational to safeguard people, ensuring safeguarding practices are followed and embedded within personal practice

      Work in collaboration with the Primary Care Network in Oldham West and other agencies across Health and Social Care to develop bespoke care plans

      Represent the organisation and our values during your work with CYP and families, at local events and celebrations as well as at relevant meetings such as CIN, Child protection, team around the family.

      General duties:

      Use IT systems including email, online systems, and Microsoft packages to record and share information.
