SCOPE OF POSITION: Responsible for performing selected nursing and administrative/clerical tasks in support of patient care management, following direction with assignments working independently and as part of a team to meet practice patient care needs. Must be flexible in assignments functioning under the direction of Clinical Operations Manager, and tasks delegated by RNs and providers.
RESPONSIBILITIES:
Responsible for initiating and performing appropriate treatment according to the patient’s needs, clinical policies and procedures, and standards of the profession.
Must be able to prepare patients for examination and treatment, taking vital signs, entering medications into the EMR, and assess the patients need for refills, initiate cardiovascular risk forms, and other treatment plan forms, perform finger stick INRs and EKGs per protocol.
Must be able to provide patient education in person and via phone to patients, family, significant others regarding medication, test results, treatment plans, symptoms to monitor, appropriate testing follow up as necessary as directed by provider or RN in accordance with HIPAA guidelines.
Must be cognizant of the continuous need to reassess treatment plans and appropriately refer to registered or provider and questionable issues.
Must be able to review correspondence and recognize need for additional follow up, orders to be obtained and or scheduling office visits as appropriate.
Handle refill requests and prior authorizations of medications per protocol, including assessing for appropriate follow up testing, and compliance with follow up visits as defined by protocol and as directed per RN/APRN/MD.
Be able to provide general information on department policies and procedures as appropriate to answer patients questions or concerns.
Must be able to utilize non licensed staff appropriately. Through good communication skills, be able to delegate and provide guidance to medical assistants assisting in patient care services.
Maintain confidentiality and comply with HIPAA requirements.
Must be able to identify limitations and seek advice of the RNs, APRN, MDs and clinical supervisors as appropriate.
Maintain skill set and participate in professional development to keep abreast of current changes and trends in the industry.
Any other duties as required based on the needs of the patients and practice.
REQUIREMENTS:
LPN licensure state of CT
Cardiac care experience a plus
Good organizational skills and thought process.
Must be pleasant, professional, articulate and have pleasant phone skills
Ability to communicate effectively with patients, families and co-workers
Must be self-disciplined and be able to work independently and as part of a team.
Second language a plus
Must be able to accept additional responsibilities as appropriate.
