What to Expect (Job Responsibilities):
- Coordinate multiple aspects of care for patients with preventative care needs and chronic diseases
- Order laboratory tests or imaging exams per protocol to facilitate patient screenings
- Serve as a liaison between healthcare providers, clinics, patients, and various departments for population health programs
- Ensure an integrated approach to patients' care coordination
- Promote a team concept and reflect the mission and philosophy of the organization
What is Required (Qualifications):
- High School Diploma or GED equivalency
- 1+ years of experience as a Certified Medical Assistant in a primary/specialty care setting
- Completion of an accredited Medical Assistant program and registered with State Board or certification through a nationally recognized Medical Assistant program
- Nationally Certified Pharmacy Technician or registered with the State Board
- Strong communication and organizational skills
