What to Expect (Job Responsibilities):
- Make clinical level of care determinations based on discussions, medical records, and other pertinent clinical data
- Utilize various tools and methods to provide patient options and education regarding care services
- Navigate healthcare options, post-acute offerings, Medicare coverage, and billing issues
- Communicate effectively with empathy over the phone and while interacting with others
- Maintain knowledge of Gentiva services including home health, hospice, and palliative care
What is Required (Qualifications):
- Associate degree required; Bachelor's degree preferred
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with professional licensure in a compact state preferred
- Minimum of 3 years’ post-acute experience; home care and hospice experience preferred
- Strong analytical and problem-solving skills
- Excellent computer and time management skills
How to Stand Out (Preferred Qualifications):
- Nursing background working across multiple areas of post-acute care
- Ability to learn and master information related to locations and services of clients
- Good working knowledge of home health, hospice, and palliative care services
- Exceptional interpersonal skills
