Job Title: Clinical Health Coach

Job Description

Our integrative care model combines the best aspects of conventional medicine with complementary approaches. This comprehensive approach to health and wellness focuses on the whole person and the contributing factors rather than a single complaint or symptom. It is simple, complete care for chronic conditions that affect quality of life. Coaching is an integral part of the client experience. We are looking to hire a health coach who can deliver a best-in-class, medically-oriented coaching experience, helping our clients achieve the best possible outcomes toward better whole-person health. While this will be a part-time job at the outset, there is a path to full time as we continue to launch with additional employers and your caseload grows.

Responsibilities

• Manage programs for a member panel with musculoskeletal pain, sleep, digestive, mental health, cancer survivorship, and weight and diabetes management issues to help them achieve better results.
• Conduct telehealth visits focused on behavioral coaching and company program adherence.
• Review health content for quality and patient understanding and provide feedback to other healthcare professionals.
• Collaborate with other coaches and team members.
• Utilize your specific clinical training and background to help define best standards for coaching and delivery of a novel digital health product.

Essential Skills

• NBC-HWC Coaching Certification
• Bachelor’s degree in health sciences, nursing, advanced practice (NP, PA), behavioral psychology, nutrition, physical therapy or equivalent.
• 3+ years experience in direct patient care in settings such as physical therapy clinics, cardiac rehab, inpatient hospital settings, outpatient hospital settings, or nutrition counseling.
• Excellent communication and written skills.

Pay and Benefits

The pay range for this position is $30.00 - $45.00/hr.
