      What to Expect (Job Responsibilities):
      - Timely acceptance or decline of referrals based on predetermined criteria
      - Establish and maintain communication with clients, family members, and referral sources
      - Partner with agency staff and sales to achieve optimal patient satisfaction and outcomes
      - Facilitate the collection of all necessary information and documentation for referrals
      - Adapt to changes in a fast-paced environment

      What is Required (Qualifications):
      - Current RN or LVN/LPN license in practicing state
      - Minimum of 2 years of applicable experience with a strong understanding of facility discharge processes and intake protocols
      - At least 1 year of experience in home care, hospice, or personal care services
      - Prior experience using Electronic Medical Records (EMR)
      - Proficiency in clinical applications and MS Office products

      How to Stand Out (Preferred Qualifications):
      - Experience dealing with a variety of payors in healthcare, including Medicare, Medicaid, and commercial payers
      - Ability to adapt quickly to changes in a fast-paced environment
