      Job summary

      The Sides Medical Centre are looking to recruit a motivated, committed and enthusiastic, Prescribing Clinical Pharmacist. Previous experience of working in General practice would be desirable and good communication skills are required to join our friendly practice team.

      Main duties of the job

      The role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy.

      The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all our patients and will be working with members of the wider healthcare team to improve care and support safe and effective prescribing.

      Patient facing long-term condition reviews - take responsibility for the care management of patients with chronic diseases where medicine optimisation is required. E.g., COPD, asthma.

      Patient facing clinical medication reviews - undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring. Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care.

      Management of common/minor/self-limiting ailments whilst working within a scope of Practice and limits of competence.Signposting to community pharmacy and referring to GPs or other healthcare professionals as appropriate.

      Management of repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.

      The Sides Medical Centre. The Sides is a teaching practice, based in Swinton and we pride ourselves in the development of staff, therefore, allowing us to meet the needs of our patients. We have a practice population of around 13,612 and good standing in the communitywith our patient participation group (PPG). To meet the patients need we have a large clinical team with Partners, Salaried doctors, GP Trainees, Advanced Nurse Practitioner, pharmacists and a nursing team of seven. The practice also had additional ARRS roles though the Primary Care Network (PCN) a Paramedic,First Contact Practitioner and a Care Co-ordinator and Nipps Pharmacy Team.

      • -Professional knowledge acquired through vocational masters degree in pharmacy (4 years) + 2 year pre-registration
      • training
      • -Extensive knowledge of medicines optimisation, acquired through post graduate clinical pharmacy diploma or equivalent experience or training.
      • -Holds an independent prescribing qualification
      • -Postgraduate Diploma in Clinical Pharmacy
      • -Demonstrates and understanding of, and conforms to, relevant standards of practice
      • -Follows professional and organisational policies/procedures relating to performance management
      • -Mandatory registration with the General Pharmaceutical Council
