      Job summary

      We are seeking a dedicated and skilled Advanced Clinical Pharmacist to join our healthcare team. The successful candidate will play a crucial role in providing high-quality patient care, working collaboratively with other healthcare professionals to ensure optimal health outcomes.       Main duties of the job

      Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans.

      Provide advanced clinical care, including diagnosis and management of acute and chronic conditions.

      Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.

      Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated patient care.

      Educate patients and their families about health conditions, treatment options, and preventive measures.

      Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.

      Participate in quality improvement initiatives and contribute to the development of clinical guidelines.

      Stay current with advancements in medical practices and participate in ongoing professional development.

      Job description

      Job responsibilities

    • Conduct medication reviews (face-to-face, telephone, and via digital platforms) for patients with polypharmacy, long-term conditions, or complex needs.

    • Provide expert advice to clinicians on prescribing and medicines management.

    • Support repeat prescription processes and medicine reconciliation post-discharge.

    • Review and action medication safety alerts and audits in line with current guidance.

    • Undertake structured medication reviews and deprescribing where appropriate.

    • Contribute to QOF, IIF, and other clinical quality improvement work.

    • Assist with chronic disease management and treatment optimisation (e.g., asthma, diabetes, hypertension).

    • Provide advice to patients on the safe use of their medicines, side effects, and interactions.

    • Work collaboratively with community pharmacies and local medicines optimisation teams.

    • Promote antimicrobial stewardship and best practice prescribing.

    • Support education and training of staff on prescribing policies and protocols.

      Qualifications

      Essential

      • GPhC registration as a qualified Pharmacist.
      • Strong clinical knowledge with experience in primary care, hospital pharmacy, or community setting.
      • Independent Prescribing qualification (or working towards).
      • Excellent communication, consultation, and organisational skills.
      • Ability to work independently and collaboratively within a multidisciplinary team.
      • Strong commitment to continuous professional development.

      Desirable

      • Experience working in a GP surgery, PCN, or other primary care environment.
      • Familiarity with EMIS Web.
      • Experience with QOF, IIF, or medicines audits.
      • Additional qualifications in clinical pharmacy, long-term condition management, or patient education.
