Conduct medication reviews (face-to-face, telephone, and via digital platforms) for patients with polypharmacy, long-term conditions, or complex needs.

Provide expert advice to clinicians on prescribing and medicines management.

Support repeat prescription processes and medicine reconciliation post-discharge.

Review and action medication safety alerts and audits in line with current guidance.

- Strong clinical knowledge with experience in primary care, hospital pharmacy, or community setting.

- Strong commitment to continuous professional development.
