      We are seeking to recruit a positive and self-motivated individual to take on the role of Clinical Pharmacist within our Pharmacy Team. The successful clinical pharmacist candidate will play an important role within our pharmacy team and other members of the PCN multi-disciplinary team.

      Main duties of the job

      Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients within your current competence, using your expert knowledge of medicines for specific disease areas

      Being responsible for the care management of patients with chronic diseases and undertake structured medication reviews to proactively manage people with complex polypharmacy

      Providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities

      Support the leadership team on ensuring person-centred medicines optimisation and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced services

      Support patients to take their medications to get the best from them, reduce waste and promote self-care where possible

      Support the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and other healthcare professionals

      Being part of a professional clinical network and have access to appropriate clinical supervision.

      Contribute to clinical governance activities / clinical audit to ensure continual improvement of service delivery.

      Provide services to support the practices deliver QOF, DES, DMOS and IIF targets

      Work to agreed standards, monitor and maintain the standard of care offered.
