What to Expect (Job Responsibilities):
- Provide care coordination, case management, and care management services to Medicare members
- Support disease management programs, educating and empowering members to manage their conditions
- Manage acute and complex health care needs, including transition of care and end-of-life care planning
- Assist members with managing their health plan benefits and finding alternative services when needed
- Work within the health plan framework to improve medical expense management and care access

What is Required (Qualifications):
- Minimum of 5 years of clinical nursing experience
- Experience collaborating with physicians in patient care management
- Current unencumbered Registered Nurse License in the state of residency

How to Stand Out (Preferred Qualifications):
- Bachelor's Degree in Nursing or Health Education
- Current unencumbered Registered Nurse License in the State of Oregon upon hire
- Current unencumbered Registered Nurse License in Washington State within six months of hire
- Experience in Medicare insurance business operations
- Background in case management, including triage, referral, and discharge planning
