      Glossop PCN are looking for a Community GP to support our multi-disciplinary team providing healthcare to our housebound patients and patients in care or residential facilities across the PCN footprint, this role is being developed to integrate with the new neighbourhood aims and agendas.

      Main duties of the job

      Glossop PCN are seeking to recruit a team of Community GPs to work alongside a multi-disciplinary team providing care to housebound patients and patients in care or residential facilities, people with lower health outcomes and joined up care in the community, either in person or remotely.

      The role will incorporate the home first, community second, admission last strategy.

      This is an exciting opportunity to be at the forefront of the delivery of our new neighbourhood model, developing relationships and building stronger outcomes for the population. The role challenges the tradition routes of care and will bring us into a future of supporting our populations. It will involve true MDT working, building relationships with GP practices, voluntary sectors, secondary care, community care, schools and other other neighbourhood groups this list in not exhaustive.

      This is an evolving service, so you will have the opportunity to contribute to the development of Team Up!

      The aim for the service is to ultimately provide a holistic approach to acute on-the-day/rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

      There is also the opportunity to undertake a sessional ARRS GP role alongside this role, should you wish to also keep some more traditional surgery work.

      Glossop PCN is made up of four forward thinking and progressive practices (population 33.000) who have developed friendly and effective working relationships with each other.

      Your employer would be Simmondley Medical Practice, who operate as the lead practice for the Primary Care Network, and you would be entitled to NHS pension with 30 days annual leave pro rata plus bank holidays and study leave.

      Working in conjunction with Community GP colleagues and other Clinical Professionals you will cover service hours (initially 08:00-18:30 Monday to Friday), working across the Glossop area. The number of sessions you undertake is negotiable and will be discussed during the interview process.
