      Job summary

      We are looking for an experienced Band 3 HCA to cover 4 night shifts per week within the District Nursing Out Of Hours Service.

      The role includes caring for patients who are approaching the end of life. It involves working on a one to one basis ie spending a full shift with one patient, in their own home.

      You will be working without close supervision, and will therefore need to use your own initiative, whilst following the patient's care plan. You must also be passionate about making a difference to people's lives, helping them to live as well as possible and to die with dignity.

      As well as being able to work independently, you will possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas - if required - to meet service need.

      To join our dedicated, caring team, you will need excellent communication skills and emotional strength. We will expect you to have a talent for forging strong empathetic relationships with a variety of people, whilst providing palliative care. We will of course provide excellent on-going training and support.

      Experience of caring for people at the end of life is essential.

      Main duties of the job

      Your role will include delivering a range of nursing interventions to patients in their own homes. These may include monitoring symptoms, undertaking simple dressing and continence care, and keeping detailed records. You will also help patients to meet their hygiene and dietary needs.

      You will be adaptable, reliable and approachable, and will be expected to support your peers, the team leader and the organisation in promoting the Trusts Values and Behaviors.

      Please note whilst your main base will be at Crewe you will be required to work across the footprint of Central Cheshire Integrated Care Partnership.

      Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.

      Job description

      Job responsibilities

      1. Function as an effective member of the multi-disciplinary team2. Maintain clear concise and legible documentation adhering to prescribed standards and protocols3. Use IT systems for relevant tasks e.g. accessing patient records & results, data collections and inputting data4. Obtain consent in accordance with Trust Consent Policy5. To report accidents/incidents on clinical incident reporting system of patients or self which occur whilst onduty.6. To maintain a patient orientated approach towards care delivery, promoting a rehabilitative and self-careapproach7. Undertake phlebotomy duties as delegated and overseen by the registered nurse, working within thepolicies and procedures of the Trust.8. Undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.9. Assist Community Nursing Team in management of patients with Long Term Conditions and in End of Life.10. Recognise the need for referral and liaise with other health care and multi-agency workers as necessary11. Participate in the induction and orientation programmes for all grades of staff who require communityexperience as arranged by the Team Leader.12. To learn and maintain an up to date knowledge of the blood tests performed and select the appropriatesample tubes for the analysis requested and ensure that they are correctly labelled at the time of sampling.13. Maintain the phlebotomy stocks, keeping them clean and in good order.14. Follow Trust policy for any needle stick incidents and ensure reported accurately on Ulysses system.To dispose of all syringes, needles, etc. in accordance with health and safety procedures
