What to Expect (Job Responsibilities):
- Foster relationships with community partners, including health homes, local hospitals, and law enforcement
- Serve as a point of contact for coordinating care between opioid treatment program clients and community agencies
- Identify and access community resources to support clients' recovery
- Provide timely and accurate documentation of client interactions and outreach activities
- Attend provider and community meetings to advocate and educate on substance abuse treatment

What is Required (Qualifications):
- Valid state-issued driver's license and MVR clearance required
- Experience in substance abuse rehabilitation and/or opioid treatment programs
- Ability to travel within the region to various CMS locations
- Knowledge of community resources related to recovery and support services
- Strong interpersonal skills to interact with clients and community members

How to Stand Out (Preferred Qualifications):
- Experience working with special populations and understanding their unique treatment needs
- Familiarity with documentation systems such as QuickBase, Methasoft, and CRM
- Demonstrated ability to manage crisis situations and resolve conflicts effectively
