- Undertake specialist and comprehensive assessment of needs
- Co-ordinate care for a defined caseload
- Undertake physical health assessments and provide or support access to appropriate intervention.

Job summary
Would you like to be part of an exciting development that is taking place within Central & South Beds particularly our Dunstable community, within a new modern integrated health and social care hub?
We have numerous posts now available for a Community Mental Health Nurse
You will be part of The Dunstable, Leighton Buzzard, Ampthill & Biggleswade Community Mental Health Services This brand new hub development will be a 'one stop' hub where people can access high-quality care and services close to where they live making it accessible, convenient and easier for local residents to use. It will bring a mix of health and care professionals in a single location to promote more joined-up working between the NHS, the council, voluntary and community sectors.
The team is also made up of a rich resource of multidisciplinary professionals who support psychological ways of working. The team includes Registered Nurses, Social Workers. Doctors, Psychologists, Assistant Psychologist, Occupational Therapists, Support Recovery Workers, Peer Support Worker, Administrators and an Employment specialist as well as working closely within a designated locality with Primary care mental health link workers, the Local Authorities and voluntary services.

The post holder will work as an autonomous practitioner within an integrated Multi-disciplinary team. This will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the client. The post holder will be confident and competent in administering IM injections and fully aware of the Medicines Management Guidance & associated policies.
