What to Expect (Job Responsibilities):
- Conduct outreach to ECM providers, FQHCs, hospitals, LTC facilities, and CHW organizations to promote meal services
- Create and maintain a contact list of target organizations
- Schedule onsite or virtual in-services with target organizations
- Actively participate in community-based events and county/state webinars
- Stay informed about referral workflows and Community Supports integration

What is Required (Qualifications):
- Minimum of 2 years of community health work experience
- Proven ability to engage with community health organizations effectively
- Excellent oral and written communication skills, including presentation experience
- Proficiency in Microsoft Office Suite, including PowerPoint and Excel
- Willingness to travel regionally, up to 50% of the time

How to Stand Out (Preferred Qualifications):
- Associate's or Bachelor’s degree
- Experience working with Medi-Cal
- Background in food and/or healthcare organizations
- Knowledge or experience in social services or nutrition
