- Perform and write concise patient-specific comprehensive medication reviews (CMRs) consistent with Monogram productivity and quality standards.
- Drive meaningful medication interventions that result in medical providers’ and patients’ adherence to evidence-based medication guidelines.
- Review patient care reports, clinical notes, and other medical records as needed for drug therapy action plan development, targeted clinical outreach programs, and quality assurance purposes.
- Serve as a pharmacist resource for Monogram local POD (field) clinicians regarding medication use and Monogram MTM resources.
- Assist with the delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram clinical staff.
- Monitor important medication use trends for patient populations under Monogram’s care. Stay current with evidence-based prescribing for individuals with kidney disease and related polychronic conditions.
- Participate in POD interdisciplinary care team meetings and produce CMR documentation to be shared with appropriate treating community and Monogram Health providers.
- Implement medication optimization processes that are consistent with the needs of our clients and our patients.
- Support Monogram and pharmacy leaders as necessary with any tasks required to deliver excellent personalized medication management services to our patients and perform all other duties as assigned.
- Uphold the mission and values of Monogram Health in all respects.
- Bachelor’s degree in pharmacy or PharmD.
- Active pharmacist license and reside in the state of position.
- 3+ years previous experience (or equivalent) actively practicing as a pharmacist, preferably within a managed care, ambulatory care, or clinical practice environment.
- PGY1 residency in ambulatory care, managed care, or related clinical field.
- BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or related clinical field.
- Board certification in MTM from NBMTM.
- MTM certificate training from APhA, Powerpak, or similar program.
- Office-based if located in Brentwood, TN. Full-time remote if located in the city or state of local POD (multidisciplinary) care team with some domestic travel.

Position: Clinical Pharmacist - PA (1099 Contractor)
The Clinical Pharmacist implements Monogram’s medication therapy management (MTM) program, ensuring appropriate selection and use of medications by our patients. As a 1099 contractor, the Clinical Pharmacist will perform timely written comprehensive medication reviews (CMRs) by utilizing patient-provided medication lists, prescription claims, and available clinical information (lab values, etc.). The pharmacist will create drug therapy action plans based on findings and medication recommendations when appropriate for Monogram Health patients, then communicate these recommendations to providers and clinical staff at Monogram Health and in the community. The pharmacist will conduct telephonic consultations with medical providers when medication changes appear necessary and speak with patients about medication use concerns. The pharmacist will work collaboratively with their local Market Physician Executive (POD physician) and clinical staff (e.g., Advance Practice Providers (APPs), nurses, and social workers) to integrate medication management into Monogram Health’s overall care model and to ensure that field clinicians are deploying practices to optimize patient medication use and outcomes.
