      Job summary

      An exciting opportunity has become available in our Dermatology Department in Warrington for a highly motivated and enthusiastic band 7, Dermatology Clinical Nurse Specialist.

      The successful candidate will be responsible for providing high quality of care to patients suffering from acute and chronic dermatology conditions.

      The post holder will be responsible for providing specialist nurse led clinics, phototherapy, photodynamic therapy, patch testing, minor surgery, providing education management to individuals with acute and chronic skin conditions.

      You will be expected to communicate effectively in demanding and challenging situations and be a team player, you will be experienced in Dermatology and possess the non-medical prescribing qualification (V300). The post holder will be required to have outstanding organisational and leadership skills

      Main duties of the job

      RGN Degree in Nursing/RN Adult Nursing

      Knowledge of patients with acute and chronic skin dermatological conditions

      Mentoring, Teaching and Assessing

      In-depth knowledge of appropriate NICE guidelines relating to dermatology

      Ability to carry out nurse led clinics, UVB, Patch testing, PDT and Biologics

      Demonstrate an understanding of dermatological pathways for treating patients with acne, eczema, psoriasis, skin cancer pathways, UVB

      Extensive experience of nursing patients with acute and chronic dermatological conditions, assessing, planning, implementing, and evaluating plans of care

      Prescribing dermatological medications

      Job responsibilities

      Job Purpose:

      To improve the primary care management of patients with chronic skin conditions and improve their quality of life, providing highly specialised nursing care with expert knowledge and evidence-based practice.

      To help prevent unnecessary referrals to secondary care and provide a seamless service of care for patients and their families and carers. To work collaboratively with members of the wider multi-disciplinary team and allied health professionals providing highly specialised information and support to dermatology patients through from diagnosis, treatment and follow up care.

      Be responsible for assessing, planning, implementing and evaluating plans of care for patients referred with both acute and chronic skin conditions. To provide expert nursing advice to patients and carers ensuring their needs are met by the team. To support the clinical team to do this by leading and challenging, disseminating knowledge and sharing best practice.

      Clinical Practice

      To deliver the highest quality of care by utilising all highly specialist nursing skills and practice. To assess, plan, implement and evaluate programmes of evidence-based nursing care for dermatological patients. To work autonomously delivering individualised direct patient care.

      To address highly complex patient issues such as treatment variations, interpretation of results, safe and effective monitoring of side effects in line with local and national policy and protocol.

      To take a lead in the promotion of patient centred care and establish and maintain a supportive relationship with the patient and their family during their treatment journey. To develop motivational and empathetic training and education for the patient around their skin condition encouraging agreement and co-operation.

      To manage and prioritise appropriate referrals as per referral criteria. To triage referrals and signpost to the most appropriate clinics making clinical judgements based upon the information available.

      To establish, organise and run both complex and noncomplex nurse led clinics as required within locally agreed protocols within Primary Care Settings. To maintain and develop current expert knowledge levels in order to provide specialist dermatology nurse led clinics. Undertake thorough patient assessment, developing a clear and concise evidence-based treatment plan, evaluating this.

      To provide highly developed specialist advice on symptom management, patient monitoring and support with close liaison and collaboration with dermatology medical staff.

      To work as part of the Dermatology team within primary and secondary care, negotiating elements of care as required.

      Manage own caseload by co-ordinating appointments and review systems, ensuring continuity of care during periods of leave.

      To act within the guidelines laid down by the NMC Code of Professional Conduct.

      Undertake skin surgery utilising highly skilled and specialist knowledge to ensure patient outcomes are met, all safety and infection control measures are adhered to and best practice is maintained in line with local and national policies.

      To maintain professional accountability through maintenance of knowledge and training.

      Ensuring that Safeguarding is in place for the respective team and apply in all cases

      Leadership

      In conjunction with other members of the Dermatology team, take a lead role in the development and regular evaluation of protocols related to own work area to maintain up to date evidence-based practice within the dermatology team.

      Act as a source of specialist advice, education, highly developed specialist knowledge, counselling and support for all health professionals, patients, families/carers and other agencies with regards to chronic skin conditions.

      As an experienced clinician, be able to advise on educational opportunities that will facilitate the development and support of specialist knowledge and skills in relation to the development of clinical nursing practice and the wider dermatology team

      Take autonomous responsibility for highly specialist treatment decisions utilising evidence-based practice, national and local clinical guidance. Communicate and negotiate these with patients undertaking sensitive and highly complex treatment decisions as required. Utilise expert clinical judgement to review and lead on complex cases and treatment decisions as required.

      Plan, organise and run clinics as required displaying flexibility to ensure patient needs are met in line with service standards and clinical guidance.

      Ensure that wherever non-medical prescribing is undertaken the non-medical prescribing policy is adhered to. Maintain own personal formulary and ensure this is regularly updated.
