- Develop and expand neighborhood programs with particular focus on undeserved communities, where health disparities exist, and those at greater risk for Alzheimer’s and other dementias.
- Continuously build and maintain relationships with residents, neighbors, staff and community partners at each residential campus to continuously provide access to programs and services that address the desires and needs of the residents and program members in that community.
- Work with Volunteer Coordinator to engage volunteers to support programming.
- Ensure each of ESLC’s residential campuses and community NPs have access to health and wellness programs that align with ESLC’s standard of care (quality and customer service), meet the desires and needs of residents and program members, and support the unique needs of the neighborhoods ELSC serves. Standards include, but are not limited to:
- Health and wellness programs include fitness classes that increase strength, endurance, and balance with an emphasis on personal safety.
- Promoting the program through a tailored balance plan, nutrition education, strength training, informative presentations and other activities.
- All program participants benefit from a social network and improvement in their strength, endurance and balance—all while making life more fun and active for all participants.
- All programs are inclusive for all communities that reside within each of the neighborhoods.
- Educational programs are available to address on all aspects of healthy living through seminars, lunch and learns, trips to community events and programs, (such as art, music, and socialization opportunities).
- Hire and supervise all staff.
- Ensure programming meets ESLC standards including programs designed specifically to address the health and well-being of older adults with or at risk for Alzheimer’s and other dementias and their caregivers.
- Assist participants to locate community resources that support his/her health goals.
- Willing to travel to various sites to support and/or deliver programming as needed.
- Development, implementation and ongoing oversight of a Neighborhood Program Toolbox which will inform the work of all ESLC residential communities and community NPs.
- Work collaboratively across all ESLC departments to maximize resources and programming options.
- Implement and maintain a data tracking and reporting system that ensures all relevant participant and program data is collected and reported or available for reporting as needed to verify and support the goals of the program including verified evidence-based programming, improved health outcomes, increased socialization, and any others as they are identified. Data collection includes bud is not limited to:
- Program impacts
- Service providers
- Attendance
- Programming schedules and frequency
- Geographic scope
- Community involvement
- Participate in the Neighborhood program team meetings on a monthly and as needed basis.
- Work with the Neighborhood program team to establish best practices for membership applications, letters, and payments for all sites.
- Ensure that health assessments and other evaluation methodologies are being utilized to gauge success of the program and impact on participants.
- Collaborate with the marketing team to ensure broad promotions of the programs.
- Manage within budget parameters and grant guidelines.
- Identify funding needs and work with the development team to match funding and grant opportunities with needs.
- Bachelor’s degree in wellbeing related field, such as health and fitness, community development, recreation therapy, social work, nursing, or physical or occupational therapy, or five years of experience in providing health and wellness programs for seniors.
- 3-5 years management experience.
- Current valid NY State driver’s license.
- Flexible, yet organized, with the ability to exercise independent, sound judgment.
- Must be knowledgeable about community resources.
- Excellent communication skills
- Outstanding leadership skills
- Knowledge and experience with Alzheimer’s and other dementias a plus.
- Demonstrated ability to collaborate effectively in a team setting.
- Ability to maintain effective and professional relationships with residents and neighbors
- Experience facilitating health and wellness group programs
- Ability to work with residents and neighbors by telephone or in person.
- Experience using modifications to work with participants who have physical health barriers.
- Experience with community based and other social support services as well as healthcare services that respond to the individual’s needs and preferences and contribute to achieving the individual’s goals.
- Ability to develop goal plan based on assessment.

Pay Range: $82,000 - $86,000 Annually
The Director of Community Dementia Services is responsible to create, maintain, and provide leadership to build neighborhood programs that will improve and enhance the health and well-being of our community’s older adults affected by Alzheimer’s and other dementias and those at greater risk for Alzheimer’s by ensuring access to ESLC’s high quality dementia care and support. The Director of Community Dementia Services is further responsible to work with community partners, ESLC staff and volunteers to deliver programming that promotes physical, mental, emotional, social, and spiritual health.
