      Valley Manor Care Center is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.

      Schedule: Monday - Friday 8am-5pm.

      The Memory Care Director will support the Executive Director in the care and treatment of residents entrusted to the facility. To serve as a liaison and role model to the community for the facility. Represents the Social Service department, other care center staff and the facility as a whole to the community. Upholds the values and mission of Volunteers of America National Services to peers and the public. Customers include residents of the facility and their families, local healthcare professionals who work with the facility and staff, and the facility staff managers.

      QUALIFICATIONS:

      • Bachelor’s degree in social work, Therapeutic Recreation, or related degree in social sciences, with current state licensure.
      • Minimum of one year of experience, preferably in a long-term healthcare setting; has knowledge of medical and public health settings, principles of public welfare and knowledge of social and health resources of the community.
      • Must be able to communicate and work effectively with various levels of staff, residents, family members and the public.
      • Must be able and willing to work flexible hours, such as during evenings and weekends, for family meetings and conferences; educational programs, admission inquiries or admissions on as needed basis.
      • Must be able to handle stressful situations, often dealing with tragic circumstances.
      • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

      ESSENTIAL FUNCTIONS:

      • Conducts pre-admission and admission procedures for residents of Memory Care Community including psychosocial assessments and monthly documentation.
        • Plans and initiates hospital or in-home visits with prospective residents on an as needed basis.
        • Meets with new residents and assists with resident orientation and adjustment to their new environment; identifies and provides for the medically-related social, emotional, spiritual and psychological needs.
        • Completes admission documents with resident, family member and/or responsible party.
        • Advises residents and families of advance directives policies and procedures.
        • Assists with the prompt referral to an appropriate agency or government program if financial assistance is indicated; complete initial certification and re-certification papers and other forms to maintain status of residents in government programs such as Medicare, Medical Assistance, Social Security and S.S.I.; notifies Business Office of any changes in financial status of residents.
      • Conducts pre-admission procedures, inquiries and tours on an as needed basis.
      • Participates in routine, or requested, multi-disciplinary care conferences/family meetings held for residents.
        • Provides support services and conducts or assists with therapeutic group work for residents and/or families when appropriate.
        • Conducts or arranges for casework as assigned; follows through on referred problems or psychological needs of residents.
      • Coordinates a community advisory committee to evaluate the appropriateness of each placement, program content and to make recommendations on program services.
      • Initiates reporting and documentation procedures for assigned residents.
        • Maintains accurate and appropriate social services documentation in the medical records for Memory Care Community residents according to state and federal guidelines.
        • Completes psychosocial assessment, i.e. social histories on new admissions; updates assessment at regular intervals in accordance with facility policies.
        • Prepares written summaries of family conferences, other related conferences, referrals made, contacts with residents and families and discharge planning in the Social Service Progress Notes in the medical record.
        • Assists with the proper notification and documentation of resident transfers within the facility and/or to an alternate setting.
        • Maintains accurate documentation on activities in the medical record.
        • Track all behavior problems and works with the nursing staff to alert physicians to behavior management problems.
        • Assists with resident discharge planning and proper documentation.
      • Plans, directs and implements activities and recreational programs that meet the specialized needs of the residents in the Memory Care Community.
        • Develops and documents an individual resident activity plan.
        • Assists the Activity Director in coordination of volunteers who help in the Memory Care Community.
      • Assists the Executive Director with the development and promotion of positive public relations and marketing through contact with social service personnel in area hospitals, welfare departments and the Social Security Administration.
        • Assists with the maintenance of current Community Resource File.
        • Provides community resource information to residents and families relating to alternative healthcare services and facilities, community health education clinics, etc.
        • Assists with planning and presenting sessions to civic groups, youth groups, schools and church groups on issues related to aging and adjusting to nursing home residence.
        • Provides community understanding of Memory Care Community through a monthly support group for persons affected in some way by dementia.
      • Notifies the Business Office of payor status changes for designated residents.
      • Administers personnel functions of Memory Care Community.
        • Work with Director of Nursing and Human Resource Representative on selection of Care Companions and nurses to be assigned to the Memory Care Community.
        • Interview all candidates for positions on the Memory Care Community and recommends only those persons adaptable for work with persons with dementia.
        • Assists with providing all employees in Memory Care Community with specialized training and orientation.
        • Works with supervisors to discipline employees on infractions of policies and procedures, recommends termination or transfers when needed.
        • Participates in the development, implementation and maintenance of work routines and staffing schedules.
      • Monitors monthly program expenditure to budget.

      Other Primary Functions

      • Advises and recommends solutions to the Director of Social Services, Executive Director, and Director of Nursing of major issues or concerns expressed by residents or their family members.
      • Performs other duties or special assignments as directed by the Executive Director.
      • Ensures residents have necessary and adequate clothing, health aids, etc.
