Job summary
This is a new and exciting role within the elective surgical division to help and support the nursing team in the safe discharge of patients.
Within this role you will be expected to communicate and liaise with the wider multi-disciplinary team to enhance and support patient outcomes, day to day activities will include speaking with patients and family members and including them in the decision making process in relation to discharge planning. Discussing with doctors and allied health care professionals to ensure patients are safe at the point of discharge by making relevant referrals to appropriate agencies and supporting the nursing team in aiding and planning discharges across the elective environment.
An additional aspect of this role with be to support the senior management team in ensuring high standards of care and safety are maintained by supporting staff and helping with the general upkeep of the ward.
Main duties of the job
Main duties included in this role will be speaking with patient and relatives to understand and achieve positive outcomes. Making onward referrals to outside agencies such as social care direct for care packages, district nursing teams for ongoing care following discharge and making follow up appointments.
Communicating with allied health professionals (physiotherapists, Occupational Therapist and social workers) as well as liaising with the discharge management team (DMT) to facilitate the step down of patients to other hospital environments.
Furthermore this role will include helping with the general up keep of the ward, assisting in topping up and keeping the ward clean and tidy, ensure safety checks are done daily and supporting the management and nursing team in additional tasks to enhance and support patient safety and outcomes.

This is a new and exciting role to work as part of the elective surgical team, due to a fast turnaround of patients elective surgery can be very busy and this new role will help support the team and patients in safe and timely discharge plans.
Part of this role will include liaising and discussing with members of the multidisciplinary team to encourage positive patient outcomes by using excellent communication skills, the successful applicant with work with a rounded knowledges of hospital professionals to support the best patient outcome.

There are many services at our disposal within the hospital to enhance the care and support patient receive at the time of discharge home and within the NHS we pride ourselves on planning discharges at the point of admission to prevent delays and unexpected hospital stays.
Written and Verbal communication skills are imperative and we are looking for a highly enthusiastic person who is able to work independently as well as part of a team to communicate effectively and make decisions in relation to discharge.
At times this role may include attending multi-disciplinary team meetings or discussions with palliative care services as well as undertaking and facilitating complex discharges, in addition to the discharge facilitator role the successful applicant will be expected to undertake general housekeeping tasked and support the ward and management team in the general tidiness of the ward, patient safety initiatives and ensuring areas are stocked up with equipment.
