      Discharge Planning Assistant, Full-Time, First Shift, Care Management

      UC Health is hiring a full-time Discharge Planning Assistant for the care management department for the day shift at University of Cincinnati Medical Center.

      The Care Management Discharge Planning Assistant will support the Daniel Drake's Center Care Management department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.They will also plan and facilitate safe discharges for patients utilizing clinical information and effective interpersonal skills.

      Qualifications

      Education and Experience Requirements:

      • Education: High school diploma or GED (required), bachelor's degree in social work or nursing (preferred)
      • Experience: 1 to 2 years (preferred)

      Hours: Full-time, 40 hours/week

      Shift: 7:30am-4pm / First Shift

      Responsibilities

      Job Responsibilities

      • Collaborate with the social worker in developing the plan of care, make appropriate referrals and coordinate the ongoing needs of the patient.
      • Secure order from physician; monitor form completion; ensure home care set-up is consistent with what is ordered.
      • Communicate with home care and durable medical equipment vendors to query patient benefits, and determine specifics of time and date that care and/or equipment is needed to maintain continuity of care for patient needs.
      • Obtain insurance approval for services as needed; charity care negotiations; special contracts.
      • Obtain and maintain current, accurate information regarding community DME, infusion and other homecare resources. Employ effective negotiation skills in order to secure the most appropriate and affordable home care resources for the patient.
      • Facilitate the discharge plan by entering patient data into Allscripts/ ECIN system for home care, DME or post-acute care facilities referrals as needed.
