Job Description

- 2 years of experience in health information management
- Experience in electronic medical record systems in a hospital setting
- Associate's degree in Health Information Management or related field
- High school diploma or equivalent
- No professional certifications required
- Proven communication skills at all levels and the ability to communicate information clearly and concisely in verbal and written form
- Demonstrated ability to review details and documents without errors
- Reviews inpatient and outpatient documentation to ensure the accuracy, timeliness, and integrity of medical record data in accordance with regulatory, accreditation, and quality standards.
- Completes chart analysis and reanalysis activities to identify any designated documentation deficiencies and entering/reconciling deficiencies in the Epic Systems Deficiency Tracking module.
- Assists the physician in clarifying deficiencies, providing technology support, answering questions, and following up on issues to ensure record completion and good customer service.
- Supports coding by following up on claims held, data integrity/other issues, and providing timely communication and resolution.
- Prepares physician notifications and warning letters, distributing them in a timely manner.
- Prepares monthly suspension lists by performing validation of delinquent deficiencies prior to provider suspension.
- Maintains accurate records of all activities performed as part of the suspension notification process, including logging of events in the electronic medical records database.
- Identifies documents needing another scan when image quality is poor and immediately routes the account through the chart correction process.
- Stays abreast of the appropriate hospital policies and medical staff rules and regulations that pertain to medical record documentation requirements to assure proper chart completion.
