      Job Description

      Provides quantitative analysis of electronic medical records and partners with physicians to obtain additional documentation or authentication of existing documentation in the effort to ensure a complete recorded history of episode of care in the patient’s medical record.

      Experience

      • 2 years of experience in health information management
      • Experience in electronic medical record systems in a hospital setting

      Preferred Qualifications

      • Associate's degree in Health Information Management or related field

      Education

      • High school diploma or equivalent

      Knowledge, Skills, and Abilities

      • Proven communication skills at all levels and the ability to communicate information clearly and concisely in verbal and written form
      • Demonstrated ability to review details and documents without errors

      Job Responsibilities

      • Reviews inpatient and outpatient documentation to ensure the accuracy, timeliness, and integrity of medical record data in accordance with regulatory, accreditation, and quality standards.
      • Completes chart analysis and reanalysis activities to identify any designated documentation deficiencies and entering/reconciling deficiencies in the Epic Systems Deficiency Tracking module.
      • Assists the physician in clarifying deficiencies, providing technology support, answering questions, and following up on issues to ensure record completion and good customer service.
      • Supports coding by following up on claims held, data integrity/other issues, and providing timely communication and resolution.
      • Prepares physician notifications and warning letters, distributing them in a timely manner.
      • Prepares monthly suspension lists by performing validation of delinquent deficiencies prior to provider suspension.
      • Maintains accurate records of all activities performed as part of the suspension notification process, including logging of events in the electronic medical records database.
      • Identifies documents needing another scan when image quality is poor and immediately routes the account through the chart correction process.
      • Stays abreast of the appropriate hospital policies and medical staff rules and regulations that pertain to medical record documentation requirements to assure proper chart completion.
