What to Expect (Job Responsibilities):
- Develop individual job plans in partnership with persons served, case managers, and local employers
- Provide education on disability awareness and assist employers in removing barriers to employment
- Offer initial training and ongoing support to individuals after job placement
- Maintain accurate documentation of individual employment plans and progress
- Assist with personal care and coordinate transportation needs for persons served

What is Required (Qualifications):
- Bachelor's degree in a related field or Associate degree with a minimum of 2 years of relevant work experience
- Strong interpersonal skills to communicate effectively with individuals and community partners
- Proficient in Microsoft Office applications
- Familiarity with the Department of Vocational Rehabilitation is a plus
- Ability to travel throughout the Dane County area, including locations not served by public transportation
