      This is a statewide recruitment, and the position may be located in the following locations: Sacramento County, San Francisco County, Fresno County, Orange (Santa Ana) County, San Diego County, and Los Angeles (Los Angeles / Burbank) County.

      The Population Health Management (PHM) Division within the Department of Health Care Services (DHCS) Quality and Population Health Management (QPHM) Program leads population-focused healthcare policy development, implementation, and monitoring at DHCS.

      As a subsection of the California Advancing and Innovating Medi-Cal (CalAIM) initiative, PHM aims to establish a cohesive, statewide approach that ensures Medi-Cal members have access to a comprehensive program that leads to longer, healthier, and happier lives, improved health outcomes, and health equity.

      Under the direction of PHM, DHCS is also building a statewide data platform, referred to as the PHM Service, designed to collect and integrate disparate information to support PHM in achieving these goals. Within PHM, the Enhanced Care Management (ECM) Unit supports the design, implementation, and monitoring of of the ECM Program. ECM is a Medi-Cal benefit intended to improve care management and health outcomes of those Medi-Cal members with particularly complex needs.

      Under the general direction of the ECM Unit Chief (Staff Services Manager SSM II), the ECM Implementation Specialist (HPS I) will serve as the technical and subject matter expert on the implementation of initiatives related to data and monitoring of the ECM benefit. The incumbent will lead, independently coordinate, and manage the associated workload seeking to coordinate and review deliverables for projects and ad hoc requests for ECM Implementation Data. The HPS I will also assist the ECM Unit Chief in standardizing the ECM referral process and conducting longer-term analysis to help define what successful ECM care models/service delivery entails.

      Working Conditions

      Employee will only need to report to Sacramento no more than twice a year.

      Telework is based on operational needs for employees who can perform their essential functions remotely and/or whose duties are conducive to teleworking per DHCS' Telework Policy and in accordance with Government Code Sections 14200 - 14203 for employees residing in California.

      Should the incumbent need to report to an office, they will need to report to DHCS office location (s): Sacramento County, San Francisco County, Fresno County, Orange (Santa Ana) County, San Diego County, and Los Angeles (Los Angeles / Burbank) County.

      This position will have the ability to complete all tasks and deliverables while teleworking. The incumbent will be headquarted in the county nearest their residence. The team of professionals this position will work with are also remote-centered and teleworking.
