- Undertaking health assessments, administering medication and treatments.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes.
- Meeting the needs of patients presenting for opportunistic wound care.
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Experience working within primary care is essential.
- Evidence of clinical skills and experience in treatment room consultation including vaccinations and cervical cytology consultations.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
- Insight and understanding of current issues in nursing and the wider NHS.
- Knowledge and experience of chronic disease is desirable.
