- Managing of asthmatic, COPD, diabetes, hypertensive and CHD patients in clinic situation. Development of individual management plans; agreeing these as appropriate with the patient and other health professionals. Sharing responsibility with the GPs and practice manager for meeting QOF targets.

Job summary
Wellspring Surgery is recruiting a practice nurse for a minimum term of 6 months maternity cover. The ideal candidate will be experienced in all things treatment room and long term condition management would be very helpful. Hours are flexible, but we are looking for someone who can work 30-37.5 hours per week and one evening finish per week. There is flexibility in the start date from late July and by the first week in September.
Main duties of the job
Working in accordance with the UKCC Code of Conduct, the post-holder will
be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, immunisations and screening. The post holder will be required to accurately record keep and code in EMIS.
