      Job summary

      Interviews to take place Thursday 8th May.

      This exciting new post will work with both Social Prescribing and Restore teams, playing a key role in enhancing the service offered to those experiencing mental health difficulties. The role will work both across the primary care teams within the 4 practices and support within the community at Restore. It will support those needing more targeted interventions providing 1 to 1 support but also working closely with Restore and supporting people to access the wider Restore offer. You will work closely with the GP Practices, Mental Health Practitioner, and also the Social Prescribing team. It will be very rewarding but not without challenges with each shift likely to be completely different.

      Main duties of the job

      • Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health access and outcomes, as a key member of the PCN multi-disciplinary team. Develop trusting relationships by giving people time and focus on what matters to me.
      • Manage a caseload that will be triaged through the Social Prescribing referral process, taking on cases for those with mental health needs. Managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals accessing the service.
      • Take referrals from a wide range of agencies, including PCNs GP practices, MAST and multi-disciplinary team such as pharmacies, hospital discharge teams, allied health professionals and act on these in a timely manner.
      • Provide brief interventions to those with Mental Health difficulties and signpost to relevant services.
      • Work closely with Mental Health Practitioner (MHP) who is working within the PCN, recognising when a person needs the MHP input and when they are appropriate to be signposted into the community.
      • Represent the team at Mental Health MDTs and associated meetings.
      • Ensure processes are followed to ensure the right referrals are made if necessary.
      • Be proactive in developing strong links with all local agencies to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.

      Healthy Lifestyles Solutions (CIC) is a Voluntary Community Service organisation working for the BD4+ Community. Two key strands of the organisation which include Restore Recovery College & Social Prescribing.

      Restore is a well-respected and established College set in the heart of a busy and highly populated location of BD4. It provides a range of recovery, coaching and training services for adults within the community, predominantly for those with mental health issues. Restore is closely linked with GP practices and are hosted at Tong Medical Practice. Restore has a strong partnership working ethos, main partners include BD4Plus PCN and Community Partnership, Statutory services, Mental health, VCS sector, Bradford College, Local Authority as well as Private businesses. Working in this way, increases the opportunities available to the local community and ensures an easy route for people to get the best holistic support possible.

      This exciting new post will sit within the Restore team offering team support to the post holder. The post is funded through the CMHT Core Model.
